Every year, on the first Sunday of the year, I provide this Spiritual Health Self-Assessment tool to our church. I have been using this personally for about 10 years and started sharing it with others about 5 years ago.
Couple of key things to remember:
* Don't let this initiate a guilt trip! This is about seeing where you are growing and where you need to allow God to shine some light of revelation in your life. Don't focus on failure - focus on improvement.
* Don't worry about the "scores". Just look for the minimum factor - that area that scores lowest of all. The scores don't matter - what matters is seeing where you can or should focus first.
* Don't try to address every area at once. Gain ground in one area at a time and focus on a new area only after new habits are firmly engrained in the one you chose first.
Here you go:

Jim, This will be my 5th or 6th year with this assessment. Terrific. Praying that 2012 will be your most amazing year. Love you brother.
Thank you SO MUCH for the challenging and practical message this Sunday. I never do New Year's resolutions but this is the year that changes. However, I am reminded of this: "It is better that you should not vow than that you should vow and not pay." Ecclesiastes 5:5. Since I'm going to call this a vow and not a resolution, I will take it much more seriously and ask the Holy Spirit to enable me to fulfill my vows.
Again, thank you for your godly leadership.
Amy
